For incoming first-year students: How do I access the CeO Student Agreement form?
Incoming first-year students must login to their MyUW and locate/click on their Student Center tile.
From there, you can access the agreement in two ways: 1) from your Tasks tile or 2) from the Students eForms tile.
For incoming first-year students: How do I know if the CeO Student Agreement form was submitted successfully?
You will see a screen that shows the form was submitted. The Task will be marked complete and will no longer be visible in your “To-Do List.” You will also receive an email informing that your form is completed and submitted to CeO Admissions.

For Continuing UW students: How & When do I apply to CeO?
Our admissions application opens every spring semester for continuing UW students that are eligible for CeO. Once the application is made available, you will want to complete the form in order to apply to CeO. For more information about the application process, please refer to our Apply webpage.
Am I eligible for the CeO program?
For two of the programs in CeO, you must be either a U.S Citizen or U.S. Permanent Resident. You must also meet at least one of the following program eligibility requirements listed below. Should you not meet this eligibility, you can still apply to CeO if you feel you would benefit from the CeO community, advising, and academic support.
- Is a first-generation college student
- Meets federal income guidelines
- Is a student with a documented disability

Academic Support

How do I sign up for academic coaching through ACTS?
You will need to complete the ACTS Tutoring request form that is located under the “For DDEEA Scholars” link and click on “Request a Coach.”
I need help with writing my papers. Who can I meet with to help me?
You can schedule an appointment with our Writing & Study Skills Specialist, Dr. Shawn Peters, via Starfish. Drop-ins are welcomed.
Are there other tutoring and learning support resources?
Yes! You can find more tutoring/learning support resources on campus by using the UW-Madison Tutoring and Learning Support Resources list. Another resource is Greater University Tutoring Services (GUTS).
